The Rehab Unit at Fairview Park
The Rehab Unit at Fairview Park is a state-of-the-art, acute care, inpatient facility committed to providing a personalized, flexible treatment program for individuals who have been disabled by illness, injury, or surgical procedures. It is a 15-bed unit located on the sixth floor of Fairview Park Hospital. All rooms are private rooms with telephones and televisions provided.
Our Rehab team focuses on functional activities such as bathing, dressing, eating, and other adaptive activities. We treat patients with a combination of medical care, therapy, and education. Patients suffering from strokes, joint replacement surgery, fractures, amputations, and other disorders are examples of the types of patients we treat at the Rehab Unit at Fairview Park.
The Rehab Team
Physiatrists - Physicians who specialize in physical rehabilitation and pain management.They coordinate your needs with your primary care physician
Occupational Therapists - Therapists who work with patients on Activities of Daily Living
Physical Therapists - Therapists who work with patients to restore movement and function
Speech Language Pathologists - Therapists who work with patients on speech and language skills
Rehabilitation Nurses - Specially trained nurses to care for you during your stay
Nutritionists - Specialists who will recommend appropriate foods that will compliment your healing process
Social Worker - The SW will provide you with information and resources to assist in a smooth transition from one setting to another
Case Managers - The CM coordinates care for your needs with other caregivers and insurance companies, monitors your progress, and communicates with all concerned parties throughout your stay. The CM assists the Social Worker with discharge plans and follows your progress after discharge
All Disciplines are involved in evaluating your needs, creating a treatment plan and implementing therapies as indicated by your diagnosis
Treatment
Your physician will make a referral to The Rehab Unit at Fairview Park if they feel that you will benefit from Inpatient Rehabilitation. The Referral Coordinator, will help assess each patients rehabilitation potential and then present the case to our Medical Director for approval.

We provide therapies 7 days a week in order for patients to increase their strength, knowledge, and abilities, ensuring the patient a quick return to their normal daily routines. We also provide education to family members who will be the caregivers to our patients once they return to their home environment.
Visiting Hours
Visiting hours are as follows:
Monday - Friday 5:00PM - 9:00PM
Saturday - Sunday 3:00PM - 9:00PM
Patients participate in therapy sessions between 3 and 5 hours per day, and families of our patients attend family education conferences as appropriate.

Hospital Stay Information
- Enough comfortable clothes for several days- sleepwear, rubber-soled shoes
- Personal toiletry items
- Personal items such as eyeglasses, hearing aids, dentures and any necessary communication aids
- Insurance card for health/hospitalization
- Current medication for patients not coming directly from the hospital
- A copy of medical records for patients coming from home
